Program Rules: This membership is not an insurance plan. Participation in the discount program requires adherence to the office's rules, terms, and conditions. Membership fees are non-refundable. If payment lapses, a $150 reinstatement fee applies. Discounted rates are based on the office's Usual and Customary Rate for each procedure, with the patient’s portion due on the day of service. Membership benefits are valid for one year from the date of enrollment. Unused services expire at the end of the term and cannot be carried over to the next year. Benefits are non-transferable and cannot be used at other dental offices, combined with other coupons or discounts, or applied alongside any dental, medical, or discount plans. They are also not applicable to services covered under worker’s compensation, referrals to specialists, or automobile/accident medical coverage. Program rules, fees, and discounts are subject to change. 904 Dental Care reserves the right to modify, suspend, or discontinue this program, in whole or in part, at any time and for any reason, with or without prior notice.
